A study from the SURMOUNT-1 trial looked at this question in people with obesity and prediabetes over a long period of time.

What Is Tirzepatide?
Tirzepatide is a medication that acts on hormones called incretins. These hormones help regulate blood sugar, appetite, and metabolism. By targeting these pathways, tirzepatide can help people:

- Improve insulin sensitivity
- Lower blood sugar levels
Previous studies have already shown that tirzepatide can help prevent the progression from prediabetes to type 2 diabetes. Researchers wanted to see if these benefits could also translate into better kidney outcomes.
How the Study Was Conducted
The analysis was based on participants from the SURMOUNT-1 clinical trial. This trial included people with obesity or overweight who did not have diabetes but had other weight related health risks.
In this particular analysis, researchers focused on participants who had prediabetes at the start of the study. More than 1,000 people were included. The average participant was about 48 years old and the average body weight was around 107 kg.
Participants were randomly assigned to receive either tirzepatide or a placebo. They were followed for up to 176 weeks, which is a little over three years.
Researchers mainly tracked two kidney related markers:
- Estimated glomerular filtration rate or eGFR, which shows how well the kidneys filter blood
- Urine albumin to creatinine ratio or UACR, which indicates kidney damage when elevated
Key Findings of the Study
The results suggested that tirzepatide may help slow down the decline in kidney function.
People taking tirzepatide showed a smaller drop in eGFR compared with those taking placebo over the 176 week follow up. In simple terms, their kidney filtration function was better preserved.
Another important finding involved UACR. Tirzepatide users experienced a greater reduction in albumin levels in urine compared with placebo. Lower albumin in urine is generally a positive sign for kidney health.
Interestingly, these benefits appeared stronger in participants who already had higher albumin levels at the start of the study.
Possible Reasons for the Kidney Benefits
There are several possible explanations for why tirzepatide may help the kidneys.
First, weight loss reduces strain on the kidneys. Obesity increases pressure inside kidney filtration units, which can gradually damage them.
Second, improving insulin resistance and blood sugar control can reduce inflammation and metabolic stress.
Third, the study also showed that far fewer people developed type 2 diabetes while taking tirzepatide. Since diabetes is a major cause of kidney disease, preventing diabetes could indirectly protect kidney function.
These combined effects may help explain the positive kidney results.
Important Limitations of the Study
Although the findings are promising, the researchers also noted several limitations.
This analysis was done in people who mostly had normal kidney function at the beginning. That means the results may not apply directly to people who already have advanced kidney disease.
Also, the study was a post hoc analysis. This means the kidney outcomes were examined after the main trial was completed. Because of this, the findings should be considered exploratory rather than definitive.
More large clinical trials specifically focused on kidney outcomes will be needed to confirm these benefits.
What This Means for the Future
The study suggests that tirzepatide could have benefits beyond weight loss and blood sugar control. It may also help protect kidney function over the long term in people with obesity and prediabetes.
If future research confirms these results, medications like tirzepatide could become an important tool for preventing both diabetes and kidney disease.
For now, the best strategies for kidney health still include maintaining a healthy weight, controlling blood sugar, managing blood pressure, staying active, and following medical advice.
Tirzepatide may eventually become part of that strategy, but more research will help clarify its full role in kidney protection.
